New Patient Registration
If you live within the practice area you may apply to register as a patient with us.
Simply come to the medical centre reception bringing your NHS card. If you don’t have your card then there is a form for you to fill in instead.
Registration forms are available to download here or ask at Reception.
Once registered, you will be invited to attend a new patient health check with the Practice Nurse. You may also be invited to attend a health check if you have an on-going health condition or if you are over 75.
Online access- Register for Online Services
Patients registered with this practice are also able to have 24 hour secure access via the internet to make appointments, order their repeat prescription and view some aspects of your medical records securely. You can even cancel appointments online even if you didn't make it online! This then frees up that appointment for someone else who needs it.

Practice Area
The practice has existing patients from a wide area to the south west of the city centre. However, it will now only accept new patient registrations from the area bounded by the following roads:
Angel Street, High Street, Commercial Street, Sheaf Street, Sheaf Square, Suffolk Road, Queens Road, London Road, Broadfield Road, Abbeydale Road, Marden Road, Sandford Grove Road, Brincliffe Edge Road, Ecclesall Road, Ringinglow Road, High Storrs Road, Highcliffe Road, Hangingwater Road, Glastone Road, Snaithing Lane, Carsick Hill Road, Coldwell Lane, Hagg Lane, Long Lane, Bole Hill Road, Walkley Road, Whitehouse Lane, Infirmary Road, Penistone Road, Shalesmoor, Moorfield, West Bar and Snig Hill.
As of January 2015, all GP practices in England may register new patients who live outside their practice boundary area. This means that you may be able to register with practices such as a practice near your work. For more information click here. Ask Reception if you feel that this might be an appropriate service for you. You will be asked to attend an appointment to see a Doctor who will discuss with you your options.
