COVID Clinics

There are local clinics available for COVID vaccination.  If you haven't had your vaccination and would like one, please click on the link to grab a jab  for further details. 

Flu clinics update (08.09.21)

You will probably have seen information in the media saying that there will be a delay to this year's flu clinics across the country. Like other Practices, we received an email on Friday 3rd September from the company who provide our flu vaccines stating that due to ‘unforeseen road freight challenges’ there could be a one-to-two-week delay to when our flu vaccines are delivered. Given that we do not yet know when our delivery will arrive and there is still a chance, we may receive it in time for the first clinic on Saturday 2nd October 9am to 3pm .

We have decided not to re-schedule just yet as we want you have access to your flu vaccine as soon as possible, however, we will not invite you until we have heard our confirmed delivery date from the supplier. 

We know this is disappointing news and we still have our fingers crossed that the delivery will get to us on time. We will keep updating the news section on our website so please make sure you check here regularly and if you have not given us your mobile phone number or consented to us contacting you by text please do so – it helps us get information to you as quickly as possible. Please do not contact us to ask if the clinics have been rescheduled – we will send texts and letters once we know. Thank you.

From 26 July 2021  there will be routine (non-urgent) pre-bookable telephone appointments available to book online or via reception each afternoon.

From 19th July, nothing will change at the surgery.  Patients will still need to wear a mask, socially distance and use hand sanitiser whilst in the surgery.  Staff will continue to wear PPE.   Thank you for your co-operation.

Please if possible order your prescriptions online or call the surgery between 10.30am and 1pm only.

If you needing a non urgent call back from a clinician then please only call between 8.30am and 10.30am Monday - Friday or 1pm - 3pm on Monday, Tuesday, Wednesday or Friday.

Due to advice given by the BMA regarding the recent Coronavirus outbreak, we have temporarily had to stop our Drop-in facility, our early morning and late appointment 'extended hours' and also all future bookable online appointments.  This is for the health and safety of EVERYONE.  We apologise for this inconvenience and hope that normal service will resume in the near future.

Your Data Matters

If you do not want your identifiable patient data to be shared for purposes except for your own care, you can opt-out by registering a Type 1 Opt-out Form.   

To register a Type 1 Opt-out, for yourself or for a dependent (if you are the parent or legal guardian of the patient) (to Opt-out) you can download the form from the link above and then return it to the surgery. If you change your mind, you can also use this form to opt back in.

This collection will start on 1 September 2021, therefore if you do not want your data to be shared with NHS Digital please register your Type 1 Opt-out with the surgery at least 2 weeks before that date.

The data held in your GP medical records is shared with other healthcare professionals for the purposes of your individual care. It is also shared with other organisations to support health and care planning and research.

Type 1 Opt-outs may be discontinued in the future. If this happens then they may be turned into a National Data Opt-out. You will be informed if this should happen and if you need to do anything. More information about the National Data Opt-out and how to opt can be found here.

CORONAVIRUS UPDATE

If you haven't yet had your COVID vaccination and would like to get protected, there are still some local clinics available for you to book your appointment.  Please call 119 or visit www.nhs.uk/covid-vaccination to arrange or manage an appointment and for further information about the COVID vaccination. .

Many thanks to all our patients for helping us with the current situation by managing your health conditions at home. We want to reassure you however that if you have an urgent health need we are up and running to continue to offer normal care, but in a slightly different way.

Our access now is by telephone. Please be patient if you can’t get through - we have had a new phone system installed to make this easier.

When you speak to the receptionist they will add you to the doctor or nurse call-back list depending on your problem. A doctor or nurse will call you back on the same morning or afternoon to assess the problem. We are also able to video call those we need to see and can bring people in to be seen in surgery if their medical condition needs this.

If you don’t have a phone, and no-one who can phone for you please come to Devonshire Green so the receptionist can speak to you about how we will assess you.

Sick notes:

If you need a GP Fit note (sick note) for work we can arrange this by phone. If you are ‘Shielding’ due to your medical conditions, or ‘Self-isolating’ due to Covid 19 symptoms in yourself or someone in your household, please click here for guidance to download a letter for work regarding this. 

We will be looking over the next few weeks at ways of continuing normal workload in new ways (for example annual health reviews for Asthma/ COPD/ Diabetes/ Vascular and heart disease) and will be in contact with you about this.

If you are suffering with musculoskeletal pain, (pain in muscles, bones, joints or  tendons), we have a Physiotherapist who may be able to offer you some support.    The clinics are on Mondays, please telephone reception and ask to book a telephone appointment in advance.

The Healthier Together is a community initiative

The Healthier Together programme relies upon patients and healthcare professionals working together to improve how local healthcare is delivered. The website provides advice for parents, young people and pregnant women, and clinical resources to support healthcare professionals – which means that your child is likely to receive consistently high-quality care, irrespective of which healthcare professional they see.

Concerns regarding face coverings:

Wearing face coverings has become mandatory on public transport and comes into effect in shops on 24 July 2020. We are aware that some patients might be understandably anxious about the Government’s recent announcements around the use of face masks in various public settings.  GPs are not in a position to provide individual risk assessments or letters for patients who feel that they should be exempt from wearing a face mask in these settings. Social Prescribing Service - We're Here for You

Our Health Link Worker can support and assess your needs and provide you with a social prescription – a service or activity – that will benefit your health and well-being. The Health Link Worker will arrange an appointment with you and discuss what types of support would benefit you. With your consent they will make suggestions and/or refer you to other services that may benefit your health and well-being and lifestyle. You will be offered support tailored to your own situation and the health link worker will keep in touch with you to review your progress. Our service works across the City with SHU Primary Care Network in Sheffield.
To access the service:

Contact your GP and ask for a referral for Social Prescribing. We look forward to working with you.
