Chronic Diseases

We are keen to support patients with longer term diseases or conditions, such as:

• Heart Disease
• Atrial Fibrillation
• High Blood Pressure (hypertension)
• Stroke
• Diabetes
• Epilepsy
• Asthma
• Lung Disease (COPD)
• Chronic Kidney Disease
• Depression
• Cancer
• Dementia
• Mental Health

With most of these conditions, you will be offered a review with a nurse (and, where appropriate, a doctor) at least once a year. This review aims to ensure you are receiving the most appropriate treatment for you and gives you the chance to discuss any issues with the nurse or doctor.

The review is normally offered within a few weeks of your birthday; please feel free to contact a practice nurse if you think you are due for a review and haven’t heard from us.

Appointments with the Practice Nurse

Appointments can be made with the practice nurse for a wide variety of treatments, advice or care, including

• dressings and wound care
• ear syringing
• immunisations, including those needed for travel
• review of on-going health problems, such as high blood pressure, diabetes, asthma
• stop smoking advice
• advice on diet, weight monitoring and healthy living
• family planning
• cervical smears and well-woman care
• health checks
